Yes it's real. Basically it's an overgrowth of the skin, similar to a wart, but way more out of control and extensive.

@ blue lasers - You mean aside from this man there is another individual who suffers from the same thing?! Who is the other person? Good thing that this man is receiving treatment and I saw the second video, he looked way much better.

i think is in Romania but his "problem" it wasn't at that stage it was less and they heal him.
