Are you absolutely sure the diode negative couldn't touch the case? If it does that bypasses the sense resistor and the driver will put out the max amount of current it can. With a fully charged battery it can put out enough current to kill an m140! I know from experience.
Not 100% but I made sure to only strip enough insulation from the wires to solder them. And no more.
I guess it's a possibility... a grim one.
