Shipping internationally is a pain in the arse.
Let me rephrase: shipping outside Teh World is a pain in the ass. From a typical American perspective, that means shipping anywhere but America. For the rest of the planet, it means shipping things off-world. Unless you live in Italy or something, in which case there's no such thing as shipping anything anywhere, for reasons unknown to anyone outside Italy, apparently.
It's like these movies out of Hollywood where the world has been destroyed, so they have to get to Europe instead (Europe, obviously, not being part of 'the world' at all). No wonder few North Americans know where Canada is (hint: it's part of the North American continent).
Really, as a sex ed teacher of mine once said: if it's a pain in the a$$, you're doing it wrong.
Edit: Obviously, I do know there are Americans who have no problem with the idea that there is more to the world than North America, and that there are several Americans on this board that have been very nice to us foreigners, including shipping some pretty delicate items overseas. My thanks to those, and I'd like to explicitly note that I'm aware the above is a generalization to which there are many exceptions whose mention in the text above would ruin the pointed sarcasm.
Edit 2: I have a remailer when she's back from her vacation, if that helps anyone. Italy is still a no-go, though.
