Can you take a picture of the whole setup ? Without knowing the cause you may have the same problem next time.
By example you could put the driver in reverse in the build.

I had a diode killing fiesta this last month! Yah wasted money. That's what I need!

Just make sure you know what the problem was if possible so you can learn from it as mentioned. Nothing worse than a diode dying and not knowing the real cause. Then it's a real waste.
