if you are using separate crystals, try cooling the Nd:YVO4 and heating the KTP. Those are 'ideal' conditions.

If you want REAL power, you focus the output into the crystal. Putting the crystals right on the die gets a pretty small beam into the diode, but not as small as you could get with a lens in front. The goal is the get the most energy density into the crystal as possible.

I would worry about finding the right optics first. Getting mounts and holders is pretty easy, optics are the hard part.
