110V switchmode power supplies often have a voltage doubler as the first stage. Bypassing this stage will allow 220V operation in most cases. Can you disassemble this component and get more pictures, or are you going to take the easy (albeit heavy and expensive) way out?
18V 20A isn't really a standard value, but it would be a lot easier to replace that PSU than use a transformer.
