help, laser no longer focusing after re-assembly, just a big blob now

Oh that's funny, they used a LED board as the driver and left the LED on it.

If it won't unscrew then it's glued at that polishes/unpolished union ( black arrow ).

Yes the set screw was to hold the inner module but it's loose now, you should pull it forward ( inner module ) and tighten the set screw then replace your lens or at least inspect it and see if the glass lens is loose in the lens barrel.

p.s. Your raw output looks like a dirty window and as Jordan said it could be loose, if the business end hole is 12mm you could always gut it with a hammer and screwdriver and replace with a copper 12mm module and driver, even step up to a 9mm diode such as a NDB7875 or equivalent.

How about post a pic of your lens while holding it up to the room light.

It could also screw together at the O-ring ..... remove the O-ring and see if there is a union line there, because the 4 holes look like tool holes made for grasping the back end without damaging the coarse threads.

p.s. The 4 holes are interesting, many items that have holes like that use a spring detente that you release by pressing a 4 prong tool into the holes, but I have never seen a laser heatsink make that way, still shine a light into the holes and take a look, maybe probe the holes with a wood matchstick just in case there's a spring detente, but it would be a 1st...... hell you could try 4 matchsticks in the holes and then turn it upside down and place on a table so you can push the body down on all 4 match sticks at once and see if there is a spring detente, if so with the matchsticks in the holes, on the table, pull down on the threaded end, up on the body.
