Are you sure that the copper module in the heatsink is not spinning? this hapens to me heaps when I put too much thread tape on lens holder for a tight fit, you have to be careful if this is the case because you can snap the diode pins or wires inside!
if this piece is spinning you only need to remove the stainless steel ring and tighten the set screw, with an allen key,

sometimes I put one drop of supaglue between my lens and focus ring to stop the spinning there, but make sure you have acetone or a strong solvent handy incase you want to change lens one day
