3 combine block, whats the name?

i have this block wich can contain 3 diodes....and has room for 2 prism pairs wich can be screwd tight.
I need to know the name of this block and who made it...and then find a tech sheet or something...
from viewed from behind it can contain 2 diodes to the right, and one at the closest point at your eyes.
