the end that looks solid(i.e. no threaded part for holding the lens in) points out. it will have 2 grooves on either side for a spanner wrench. edit: in the pic the end facing you will be the end that points outward...

it takes 1.2v over the voltage needed for the diode itself. if your diode requires 3v to operate then you would need 4.2v going in to the driver.

im not sure if he used the heatsink i sent him or the rig he already had but they were heatsinked

also at that low current you can probably just use a flexdrive since the voltage will be lower at the lower current..

its the same price as a phr803 sled. you also have to note that the "series 1" factors changed a bit during testing. in the quote from him he said its better to use "series 2" as a reference.
