Yeah pictures would help us help you. Double check your soldering on the driver for a short or bad connection, one of the most common causes in your scenario is not having your -Vin on the driver grounded to the host but I wouldn't recommend testing this unless you know what you're doing not to...

If you seared some dust or dirt on the lens I highly doubt there's anything wrong with your diode, unless of course the lens mounting is poorly installed that allows tiny amounts of dirt/dust inside the head of the laser. In your last picture you can remove the lens from that mount & replace...
