Looking at your design, it occurs to me that you should see if you can find the AX5525 IC. I've never been able to find them actually for sale, but if you can find them, they're the same package and they look pin compatible with your design, and would allow you to kick the current up even...

You may have killed your diode by using an improper Chinese LED driver on a laser diode. That's not just a LED driver, it's a dirt cheap one with a low frequency IC. It's probably crazy ripply.

The version pictured used slightly cheaper TO252 diodes, and they're only rated for 15A average. Another revision that's otherwise the same swapped those for the 30A TO263 diodes. I may swap for TO220 though, with neutral tabs that can be heatsinked. Dirty PCBs. They're as cheap as it gets...

Both. By choosing a jack, you choose the number of diodes. There are also two jacks on the other half of the board for choosing between a 0.1 and 0.01 ohm sense resistor.

The underside is meant to be attached to a heatsink / fan, like a CPU cooler. Thin aluminum will be used to level the various ICs, and then arctic silvered to the heatsink.

There are probably two reasons: 1) Most adjustable drivers use some form of voltage drop scaling, and typically that involves a second IC in the output current path. That second IC is often powered directly from the output current path, which means that if the IC has a minimum operating voltage...
