    Well, considering I used a 12volt computer power supply to power mine, yes it will work at 9 Volts. If the power supply has the switch BUILT IN, Then yes, it is between the start of the circuit (the outlet) and the driver, and will work fine. Just don't add ANOTHER switch after the driver...

    No, the diode is press-fit into the module, meaning that you are going to need a lot more force than just your fingers. You can use the back section of the aixiz module to press it flush (put the pins of the diode through the hole in the back part of the module and push it together in a vice)...

    I HIGHLY doubt that laser is actually 100mW output. If you just want a pointer then maybe that one will suit your needs, but remember that 405nm is deep violet, and 445nm or 473nm is blue.     As far as the original question goes: yes, you may have toasted your diode or it may have been DOA. If you don't set your driver up with a test load (AKA Dummy load), your driver may let too much voltage through to the diode before it can adjust to the diode's needs, killing the diode. I killed...
