Might be a blown emitter, like others said they have three. I've had some success with totally dead shorted diodes by reverse pulsing them at low voltage/high current but not so high they blow out the bond wires.

Might be worth Epoxying over the delicate parts such as the SMDs. It also means that a short is less likely to result in a runaway situation, and makes modding the unit less risky.
