Smartphone Survey - Research Student Needs Help!

Hi everyone.
It's been a while since I've posted anything (Well, then again I never really posted much anyways. Usually just came by to read). Starting to get back into the hobby and I can see a lot has happened since I dropped it a few years ago. Feels weird to be posting about something off-topic as the first thing coming back. Well, anyways...

I'm helping my cousin with her senior thesis project. She's gathering data about smartphone usage and the impact it has on families. Below are two surveys, please pick the one that best describes you, if either of them describe you (or something... I have no idea how to word that. So unprofessional T.T).

Would help you out, but it's been a while since I've been a teen, and hopefully there are no teens of mine running around out in the world, that I don't know about.
