Don´t you think that this would create the feeling that the cigartette is from the third person, not the old lady?Another way to try... the arm in red, it's hand, and the cig colored and everything else (including sitting lady's hand) desaturated... so the delineation between the outside influence and the sitting lady's state is clear and the boundary touching is where the eye flows following the color into frame...

Maybe she is ready to quit life. Maybe she even started in high age as her husband passed away.Sad picture in 2 ways. First is she's still smoking at that age. Second is someone bummed a smoke from an old lady.
