Hello, I'm new to the forums, but I awoke today to find a surprising message from FedEx. I ordered two lasers from laserbtb - the HL-405-100mw(fda) and the LP-405-5mw(fda). Laserbtb shipped it on Monday, but didn't send an email until Thursday due to the recent holiday, and it was sitting in Memphis awaiting clearance then. Today it changed to Clearance Delay - import. The reasoning is:

Ship (P/U) date :
Sun 4/28/2013 2:36 pm
SHENZHEN, CNCN
Clearance delay
IRVING, TX
Estimated delivery :
N/A

Goods are subject to review by Food/Drug Agency prior to clearance.
Recommended action:
No action required at this time.
Reason for delay:
A Radiation Declaration is required.
Recommended action:
Shipper or Importer must provide a completed Radiation Declaration.

Has anyone dealt with this problem successfully? I did also send an email to laserbtb, I chose their saless@skylasers.cn address, but if anyone has the email for Kevin from the previous group buy, I could send him a copy as well.

Thank You everyone for your help!

Yes get in touch with laserbtb, they will send you the declaration form, you send it to customs, and your laser will be on its way. Same thing happened to a few in the GB.

Just got the forms, quick question what is the entry number? I also listed Memphis as port of entry since Anchorage was never listed on the tracking, or should I use Anchorage?

Thanks!

Just got in touch with the FedEx trade manager Ramona in Memphis, and she says they also need a filled out FCC 740? Anyone dealt with this yet? Last I knew, lasers were regulated by the FDA and not the FCC. Any insight is greatly appreciated!

Thanks!

Forms were sent in today, and it cleared customs today as well. In case anyone else has to fill out a form 740 for the FCC, use: Port Codes - US Customs Ports
for the port of entry box, and then "8443.32.10" is the tariff code for lasers. Leave the FCC ID field blank, unless the manufacturer actually has one, which is unlikely, then finally I chose option 7 in part 2, since there were only 2 devices in the parcel. It might be worth it to order lasers in shipments of 3 or less to make this form easier to process, otherwise you could try option 2 and see what they say about that.
