Another thing to try is to remove that focusing ring and try focusing it down by turning the lens barrel manually. Also, depending on how deep the 12mm module is, if you're using the spring that comes with the lens, it might prevent you from screwing the barrel all the way down. I've had to put some Teflon tape on the threads (ensuring that I did not have any exposed to the laser diode light) to keep my lenses from wobbling in the absence of a spring.

You might want to put the laser into a bigger heatsink too. Even though you're only putting in 100mA it might burn out.
