1W 445 driver

I got this driver from Modwerx, but before I wire it up I want to make sure I'm connecting it properly. Does anyone know where the positive and negative pads are for this driver? There are 3 pads, but none are labeled clearly. One appears to have a B+ near it which I thought might be the positive pad.

If I remember correctly, the connections are....

Edit: This is for 405nm or 445 LD

I see. Thanks that helps a lot. I guess this is meant to be in a case positive host. If I wanted to set it up as a case negative, I would need to omit the case pin when I connected it to the diode.

Edit:

For anyone else who wants to set this driver up for a case negative host, here is how I did it. The first wire coming out of the host is from the positive terminal on the battery. The second wire runs from the negative input on the driver to the case pin on the 445 diode. The case pin bent out of the way of the pad on the driver leaving the pad disconnected.
