Your diode is dead, to fix you need to replace it with a new one. But it's better to find the source of the problem before doing that or you're going to end up with another expensive LED.

Replaced diode, everything is working fine but after a period of time it still starts blinking. I wonder if the driver is overheating? It doesn't seem to be any problem untile the driver is inside the host.

That sounds quite possible if it's not heat sinked. I think those need it for anything over about an amp.
