Help me ID & repair my driver please

I got this off the board awhile back. Ive had two of these and they both have had the same qc issues this one broke both leads off as the heatsink spins .

anyways can anyone id this driver and tell me where i solder my + and - too

I also need to know how to determine my - and + posts on the 445 diode

Use silicone wires for the build. Normal plastic wires are too stiff and melt the insulation under solder temp.
