No offense, but I would think your problem is psychological. I am not saying you are crazy at all, I am however saying it is likely it is something unconscious and all of your thinking and worrying about it only making it worse. You may be amazed at the tricks your mind can play on you, it is still within logic that it could explain the symptoms ceasing when you leave your home. I would suggest seeing a doctor and describing the symptoms while holding back any theory as to the source so that you can get an honest physical diagnosis of the problem without any assumptions on their part. If nothing is found, I would then turn to a psychologist and see if they can help you discover a possible underlying reason you would be experiencing this. Again, I'm not calling you crazy, it is a legitimate and fairly common thing for someone to unconsciously react to events, sometimes traumatic or disturbing, in ways most people would just mark as being completely insane. There have even been people that basically painfully torture themselves mentally for no apparent reason but are able to overcome it through therapy.
See a doctor and then see a psychiatrist.
