Make up a company.
I can start a new business under whatever name I choose, TODAY.
As long as the company name/number isn't "claimed" (inc., Ltd., etc.) then you are fine.
If they ask for your letterhead or whatever, just make one.
If your business starts to make any money or becomes worth anything, it will cost a little over $1000 to register it in the lawyer's office but that is another can of worms.
Just make up a name, it also helps if your email address bears the companies' name as well.
Just make a hotmail account with the company name.
*edit* Your affiliation would be "Chief Executive Officer", "chief financial officer", "executive manager", etc.
